At your new patient visit, Dr. Giangobbe will perform a complete medical history, physical examination, and ultrasound examination of the legs, Dr. Giangobbe will go over his findings, discuss options for your care, and recommend a course of treatment, if indicated. He'll answer any questions you have. Plan to be in the office for about 2 hours.
Vein procedures are done using local anesthesia. The good news is that most treatments are relatively painless. Some people may experience mild discomfort, but this is usually short-lived and goes away quickly. Most patients don't need any pain medicine afterwards.
Most patients can resume their regular activities like driving, walking, and doing housework right away.

What are Varicose Veins?
Varicose veins are commonly thought of as the twisted bulging veins that can be seen on the surface of your legs and ankles. They are a sign of a medical condition called chronic venous insufficiency. Simply put, varicose veins form when the valves in your veins stop working as they should. Instead of going back to your heart, the blood in varicose veins stays in your legs. When that happens, the veins stretch under the extra pressure of the pooling blood and become inflamed. While the veins become unsightly, it's the high pressure and inflammation that cause varicose veins to be a medical problem.

Varicose vein disease isn't always obvious. Many other leg symptoms can occur without visible bulging veins being present. Have you noticed any of these?

What Causes Varicose Veins?
Varicose veins usually develop slowly, over many years. A number of factors can cause this condition. Here are some of the more common causes we see at our Sun City West and Surprise vein treatment center.
Age
As you get older, the effects of a lifetime begin to show. Almost half of people over 50 years old have varicose veins.
Family History
If you have one parent with varicose veins, there's a 50% chance that they'll pass on to you. The risk increases to 80% if both your parents have it.
Prolonged Standing
Jobs where you have had to stand or sit for many hours increases your risk for developing varicose veins.
Pregnancy
Hormonal changes cause the veins to swell during pregnancy. Afterwards, in some cases, they do not return to normal and varicose veins develop.
Leg Injuries
Whether a major accident or a lifetime of leg bumps and jostles, injuries to the legs can often lead to the development of malformed varicose veins.
Past Vein Disease
Have you had vein problems like a deep vein thrombosis (DVT) in the past? This puts you at risk for developing varicose veins.
Risks Of Varicose Veins
If you have varicose veins, you may be at risk of developing a number of complications. These can include skin problems, blood clots, and ulcers.
When varicose veins are present, the skin over them may become thin, red, and irritated. This is a condition called venous eczema which can be painful and make it difficult to walk or stand for long periods of time. If you think you might have venous eczema, it is important to see a doctor.
Varicose veins cause fluid to build up in the tissues. If swelling in your legs doesn't seem to go away, it could be a condition called lymphedema. It occurs when the lymphatic system has been damaged or is unable to function properly. In addition to chronic leg swelling, lymphedema causes pain and leads to developing chronic leg wounds and infections.
If you think you may have lymphedema, it is important to see a vascular specialist like Dr. Giangobbe so that they can make a diagnosis. There are several different treatments available for lymphedema, so there is no need to suffer from this condition
Varicose veins cause blood to pool in the legs, leading to chronic inflammation and the buildup of fibrous tissue. This hardening and thickening of the skin is called lipodermatosclerosis. Symptoms of lipodermatosclerosis include pain, itching, and burning sensation. Untreated, lipodermatosclerosis also lead to leg ulcers and infections.
Another potential complication of varicose veins is the development of ulcers. Venous ulcers most commonly develop on the inside of the leg, just above the ankle. These wounds can be painful and may take a long time to heal. In some cases, they can even lead serious infections. Fortunately, with proper treatment by a vascular specialist, the vast majority of venous ulcers will heal.
Superficial thrombophlebitis (SVT) is a condition that usually occurs in surface veins of the legs. It is caused by a blood clot that forms in the vein, causing it to become inflamed and painful. The condition is not usually life threatening, but it can cause significant discomfort and pain. If you suspect you have an SVT, it's important to have it evaluated to make sure there isn't also a deep vein thrombosis or infection.
Deep vein thrombosis (DVT) is a blood clot that forms in the deep veins of the leg. People with untreated varicose veins have been shown to have a 6x greater risk of developing a DVT. A deep vein thrombosis can lead to serious complications including a blood clot that breaks free from the vein and travels to the lungs called a pulmonary embolism (PE) which can sometimes be fatal.
If you think you might have a DVT, it's important to seek medical attention immediately.
Diagnosing Varicose Veins
Varicose veins, known medically as chronic venous insufficiency, are diagnosed using a special ultrasound examination called a venous duplex. This is a painless test that uses soundwaves to check to see how well your leg vein valves are functioning, the diameter of the veins, if there are any blood clots present, and any other venous abnormalities.
Dr. Giangobbe is one of a very few vein specialists nationwide qualified to perform his own vascular ultrasound testing. This allows him to give his patients the most complete evaluation and treatment planning possible.

Endovenous laser therapy (EVLT) is a minimally-invasive treatment for varicose veins. It uses laser energy to close off the affected vein, allowing blood to reroute through healthier veins. EVLT is a safe and effective alternative to more invasive treatments like surgery.
Radiofrequency Vein Ablation
Radiofrequency vein ablation (RFA) is a treatment option for varicose veins. It uses radiofrequency energy to heat and destroy the vein which is then absorbed by the body. RFA is a minimally-invasive procedure, meaning it does not require surgery or general anesthesia.
Clarivein
Clarivein (Mechanochemical Ablation) is a minimally-invasive treatment for varicose veins. The procedure uses a small catheter with a rotating tip to injure the affected vein, causing it to collapse. Clarivein is a safe and effective alternative to traditional varicose vein surgery.
Transcatheter Embolization
Transcatheter embolization is a procedure that is used to block blood flow to large areas of malformed veins. It is a safe and effective treatment performed using local anesthesia.
Venaseal
Venaseal is a minimally invasive treatment option for varicose veins. It uses an adhesive glue, cyanoacrylate, to quickly and effectively close enlarged veins.
Sclerotherapy
Sclerotherapy is a minimally invasive procedure used to treat spider and small surface varicose veins. A solution is injected into the veins causing them to swell shut. This seals the veins and prevents blood from flowing through them. Over time, the treated veins will fade away.
What Can Prevent Varicose Veins?
While there is no guarantee that you will be able to avoid varicose veins entirely, by taking steps to improve your overall health you can reduce your risk. Here are a few things you can do to your legs in good condition.
Exercise Regularly
Walking is one of the best exercises to prevent varicose veins and promote better circulation. Other forms of aerobic exercise, such as swimming and bicycling, are also good.
Healthy Weight
Varicose veins are more common in people who are overweight or obese. Maintaining a healthy weight can help reduce your risk of developing varicose veins.
Elevate Your Legs
Don't stand or sit all day. Elevating your legs helps reduce the pooling of blood in your legs and the risk of varicose veins.
Compression Stockings
Compression stockings apply gentle pressure to your legs helping to prevent the pooling of blood in your veins.

Mitchell J. Giangobbe, MD is a general and peripheral vascular surgeon who has practiced in Sun City West since 1995. He received his medical degree from the University of Arizona in 1990 and completed his surgery training at the University of Iowa.
Dr. Giangobbe is board-certified by the American Board of Surgery and the American Board of Venous and Lymphatic Medicine. In addition, he has specialized vascular ultrasound certification as a registered vascular technician (RVT) and registered physician in vascular interpretation (RPVI). He is a fellow of the American College of Surgeons (FACS).
A well-known surgeon in the Surprise and Sun City West community, Dr. Giangobbe was chief of surgery for many years at Del Webb Hospital. He has been featured in Phoenix Magazine's Top Docs, America's Top Surgeons, and AZ Top Docs for his surgical skills and patient care.
